Acquired epileptic aphasia with behavioral disturbance in an eight-year-old boy: A case of landau-kleffner syndrome managed with intravenous immunoglobulin

Abstract
An 8-year-old boy presented with progressive language impairment
beginning at two years of age and behavioral abnormalities for one year. He
exhibited repetitive speech, poor response to verbal commands, episodes of
abusive language, and frequent anger outbursts. Following initial assessment
and stabilization in the Emergency Department, the child underwent multidisciplinary
evaluation involving Neurology, Psychiatry, Pediatrics, Radiology, Pathology,
and Otorhinolaryngology (ENT). Pediatric assessment confirmed isolated language
delay with preserved gross and fine motor milestones. Radiological review
excluded structural intracranial abnormalities, while pathological evaluation
of hematological and biochemical investigations ruled out infectious,
metabolic, and inflammatory etiologies. ENT evaluation excluded hearing
impairment and other otorhinolaryngological causes of language regression.
Based on previous neurological evaluation and current clinical findings, a
diagnosis of Landau-Kleffner syndrome was confirmed. The child received
intravenous immunoglobulin therapy over five days along with clobazam,
risperidone, clonidine, and methylphenidate. He remained clinically stable
throughout hospitalization and was discharged with advice for repeat
immunotherapy after three weeks, continued speech-language therapy, and
multidisciplinary follow-up.
